Jersey Deposit - Competitive - by checking these boxes, you CONFIRM your understanding of the following:

Team representatives hereby acknowledge that the deposit is only a usage deposit and does not constitute the replacements costs of the jereys and/or sponsor tags.
Competitive - the $1000 jersey deposit will be added to your ice bill in Nov/Dec.

Jersey Deposit - House League - by checking these boxes, you CONFIRM your understanding of the following:

Team representatives hereby acknowledge that the deposit is only a usage deposit and does not constitute the replacements costs of the jereys and/or sponsor tags.
A Jersey deposit of $1000 ($500 for U9 and U11) payable to the "NMHA" by team cheque is due to the office by Dec. 1, 2022. If the deposit is not received on time, practice ice will be withheld euntil payment is receivd.

Jersey Return - by checking these boxes, you CONFIRM your understanding of the following: *

All jerseys and sponsor tags must be accounted for at the end of the season. At the end of the season, all jerseys are to be returned WASHED and/or laundered and on hangers in the sweater bags provided. Any missing and/or damaged jerseys/tags will be charged to the team at replacement cost per SET.

Any jerseys returned showing signs of glue, tape or any adhesive that was used to attach tags and/or letter "C" or "A" will be deemed as damaged and the team will be charged for the full price of a set of jerseys for each set that is damaged.

Team representatives picking up the jerseys must check of the jerseys within 48 hours of receiving them for any previous damage. Should there be damage to one or many jerseys, the team must fill out a sweater report online within those 48 hours.
